Below you will find our 2022 – 2023 Application for Enrollment.

Please complete the appropriate forms for your child/children and return it to the main office with the appropriate registration and book fees.

Students who:

 

                     Will Return:                  Please complete the “Application for Enrollment” and “Immunization” forms

                     Will NOT Return:         Please complete the “Authorization for Transfer” 

 

 

Summer School:          Students currently attending, please complete the “Summer School” form.

 

 

Summer School:          New Students please complete the “Summer School” and “Immunization” forms.

For students coming from other schools, K Anthony may verify the child’s previous school’s academic performance and discipline status.  Finally, the application will be reviewed by our Directors to determine your child(ren’s) compatibility for placement.  Parents will be notified of the admission decision.
